Iowa Transition Assessment
Welcome!
This website provides resources and tools to assist in the identification of the interests, preferences, strengths and needs of students with IEPs, so that the student will be better prepared for employment, further education and independent living. The information on this website has been developed by a team of educators from Area Education Agencies, Local Schools, University of Northern Iowa and University of Kansas, and the Iowa Department of Education.
The
Iowa Model for Age-Appropriate Transition Assessment
provides the process and questions to ask when determining the interests, preferences, strengths and needs of a student.
The
Matrix for Transition Assessment
provides the framework by which to gather necessary information for living, learning, and working combined with student interests and preferences; basic skills; attitudes, habits, self-awareness; and critical thinking, application skills. The Matrix is divided into cells which provide hyperlinks to recommended assessment methods and potential tools.
Methods and Tools
include formal and informal assessment tools, a description of the formal tools, and information on where to obtain them. PDF and web links are included within the cells. While the hyperlinked cells provide many options, they do not provide an exhaustive listing of all the tools that may be available.
Documentation of the Assessment Information on the IEP provides instructions and examples of how to accurately document the information, gathered through the transition assessment activities and tools, on the student’s IEP.
